The incident resulted in the injury of 35-year-old Samuel Cook. Cook is a convicted felon who is prohibited from owning firearms. Based on his actions before and during this critical incident, Cook was arrested for Attempted Murder of a Law Enforcement Officer, Aggravated Assault on a Law Enforcement Officer, and Possession of a Firearm by a Convicted Felon.
Around 6:40 p.m., District 1 patrol officers and officers with JSO’s Citywide Task Force responded to a home on West 22
nd Street. The complainant reported individuals at the home were dealing drugs. The complainant also reported at least one person at the house had a firearm. Officer Mills and Officer Lyons parked outside near the home in an unmarked, covert vehicle.
Around 7:15 p.m., Cook approached the vehicle the two officers were in from the back. Cook walked around to the front of the vehicle and pointed a gun at the two officers. Officer Mills and Officer Lyons used their JSO-assigned handguns and fired at Cook while they were seated in the vehicle. Cook then began to run eastbound on 22
nd Street. The two officers exited the vehicle and ran after him.
Cook shot at the officers multiple times while he was running from them. Cook dropped his gun on West 23
rd Street, where he was taken into custody. Officers provided emergency aid to Cook until paramedics arrived to take Cook to the hospital. At the time of filming this critical incident briefing, Cook was in stable condition.
A firearms expert forensically examined the handgun Cook fired during this critical incident. From the examination, the expert determined the gun had been used in a shooting in November of 2025.
The State Attorney’s Office will independently review the incident to assess the legality of the officers’ actions. JSO will then conduct an internal review to ensure the officers acted within policy. Conclusions remain pending.
