Registry for Endangered, Vulnerable and Missing Persons (R.E.V.A.M.P.)
R.E.V.A.M.P. Duval is a voluntary identification program that aids in the safe return of individuals that may be endangered, vulnerable or missing with cognitive and memory impairment or sensory disabilities. This program was created in partnership with the City of Jacksonville, Jacksonville Sheriff's Office and the Jacksonville Fire and Rescue Department, and the registry is free to those who apply.

- If you would like to register by phone, you may contact the City of Jacksonville's Emergency Preparedness Division Office by phone 904.255.3172. Please note you may be asked to submit a photo of the individual by email or mail.
