You are about to migrate away from the Jacksonville Sheriff’s Office to the Florida Department of Highway Safety and Motor Vehicles (FLHSMV)'s website that provides the Online Driver Report of a Traffic Crash.

Before proceeding to the State website to report the crash, please make sure that you have a completed crash form provided by a law enforcement agency. It includes information about the parties involved in the incident. This can be obtained on scene, or from law enforcement at their offices if you have the required information about the other vehicle and driver, if applicable.

You are required to self-report within 10 days of the incident. (F.S. 316.066) Your auto crash will need to meet the criteria set forth by the Florida Department of Highway Safety and Motor Vehicles which is listed on the online submission form.
